Accommodation Support

We offer a a range of Accommodation Support:

  • Individual Options:

    Individual options are designed around the clients individual accommodation needs and usually involve just the one person with a disability living by themselves in the community with Support Workers assisting in the areas identified by the client and significant others.

    All individual options are managed by professional case managers who co-ordinates the support services as well as maintaining community links.

  • Cluster Accommodation:

    Cluster accommodation offers independence and privacy while providing 24 hrs support. Clients live in their own unit / apartment with support staff moving between apartments as needed. Support staff sleep on site and are therefore available at night in case of emergency.

    Small groups of units provide the ideal balance of independent community living, coupled with security and support.

  • Group Accommodation:

    Group accommodation is a good option for those people who do not want to live alone and who require support.

    It is common for people to live in a house together eg, families, flat mates etc; so it is with group accommodation. Many people want to be surrounded by others to provide company as well as sharing resources and the cost of living.

 
Case Management

Case management involves the co-ordination of services that makeup the support network which facilitates independence, health and well-being and general quality of life of the person with a disability.

Case managers are professionals who become the central point of contact regarding the service being provided. They work with the person with a disability and significant others (family, legal guardians etc) on producing the individual service plan which is central to the support they wish to receive. This plan is officially reviewed yearly however, service changes occur as required or your circumstances vary.

Case managers formulate support plans based on the individual service plan. These provide the support staff with the knowledge they need in order to fulfil the needs and supply a quality service to the person with a disability.

Medico-Legal Assessment

Staff at TIS include Occupational Therapists who are able to assist with medico-legal assessment.

We are approached by lawyers either representing the person with a disability or the Insurance Commission of Western Australia. Staff at TIS formulate and carry out professional, honest assessments of the needs of the person with a disability.

Assessments can occur as 1:1 by the Occupational Therapist (OT) or a combination of OT and Support Worker involvement over a period of time to:

  • Try and make rehabilitation gains and formulate appropriate systems and strategies to support the person with a disability in the future.
  • Develop a true objective picture of the persons actual needs over a reasonable time frame which includes specific areas the person with a disability needs assistance, type of assistance required and possible alternatives to 1:1 direct care support to achieve pre-incident skill levels.

TIS can give a costing on the service required; but the report can then be presented to other organisations for quotes for service by either the person commissioning the report, or TIS can do this on their behalf.

TIS is capable of taking on the long term direct support & case management service for the person with a disability should they wish it.

Intensive Family Support

Intensive Family Support provides services to the person with a disability who is living at home with others, e.g.. parents, spouse or children etc.

Support Worker assistance in the family home combined with case management services provides a co-ordinated support system to enhance the quality of life for all concerned. This service can have a support worker providing assistance several times a week, or at various times throughout the year.

As with accommodation support, the case manager assists in formulating the individual service plan with the person with a disability and significant other to identify the type of assistance you wish to receive.

Alternatives to Employment

The Alternatives to Employment service provides the person with a disability with alternatives to paid employment. The emphasis is on skills development while actively participating in both their local & general community.

Activities often have more than one focus. For example a simple trip to the shops can involve planning a route & taking public transport, money handling, communication & social skills. The more practiced anything is the less stressful it becomes and the more confident and happy people are.

Individual Service Plans are utilized to identify goals and setting direction to achieve those goals.

Post School Options

The Post School Options services are similar to the Alternatives to Employment services, but are focused on those who have left school within the last 2 years and who:

  • Are unable to work for 20hrs or more per week in open employment.
  • Are unable to partake in full-time study
  • Unable to achieve a combination of the above.

Individual Service Plans are utilized to identify goals and setting direction to achieve those goals.

Identified goals are generally in the areas of:

  • Social participation.
  • Developing social independence.
  • Contributing to life long learning.
  • Developing & maintaining natural supports.

 

Community Living Program

The Community Living Program is a flexible program funded through the Disability Services Commission of Western Australia.

The idea is to make funding available which can be utilized in a variety of flexible ways in order to give the person with a disability the ability to enhance their life at home and in the general community together with their well-being.

It offers a non-traditional way of utilizing the funding and can be used to purchase things like services and equipment.

This program is similar to the Family Living program, however, it is for those people who are living in teh community independently from the family home.

Family Living

The Family Living Program is a flexible program funded through the Disability Services Commission of Western Australia.

The idea is to make funding available which can be utilized in a variety of flexible ways in order to give the person with a disability the ability to enhance their life at home and in the general community together with their well-being.

It offers a non-traditional way of utilizing the funding and can be used to purchase things like services and equipment.

This program is similar to the Community Living program, however, it is for those people who are living in a family home.

Disability / Justice Programs

Disability / Justice Programs are designed to support people with a disability who are re-entering the community from a justice facility.

This service incorporates Support Workers and a Case Manager working together with community based services, such as and Department of Justice to assist the person with a disability to re-enter society.

A support network is created and strategies initiated to:

  • Assist the client become a constructive member of their local & general community.
  • Change or lessen behaviors that may lead to re-offending.
  • Provide company, companionship and role modelling.
  • Assist in creating a new social network.
  • Assist in identifying and achieving realistic goals, thus increasing self esteem and self worth.
Self Managed Options

The Shared Management Model is based on the principle that people with a disability and or their families / carers are the natural authorities about their own lives and have the right to be involved in the decisions about their services and supports.

Individuals meet with their chosen service provider to work through some guides. These assist in forming a checklist and a record of who will take responsibility for each task. An agreement is then signed by both parties detailing who is responsible for each task. The split of the tasks determines if the arrangement is self or organisation managed.

Some individuals will want to become heavily involved and take a lot of responsibility, others will wish to play a smaller role.
